Dog Park Renewal Application

All-Access Dog Park Passes will allow access to the Mack Avenue Dog Park and the Lake Front Dog Park.

  • Dog Park Pass Renewal fee is $40 per household. There is no charge to renew a Lake Front Park Only Dog Pass (LFP-only).
  • All residents renewing their Dog Park Pass/LFP-only Dog Pass must have current required vaccinations*, homeowner's insurance**, and GPW Dog License on file with the Department of Parks & Recreation. If there is any lapse in coverage, current documentation must be provided before the renewal will be approved. 
  • All residents renewing their Dog Park Pass must agree to the Lake Front Park and Mack Avenue Off-Leash Dog Park Rules.
  • A limit of 2 dog per adult when visiting the park. Up to 3 dogs in a household can be registered on the same pass/fob.
  • Dogs with valid Dog Park Passes or LFP-only Dog Passes may be walked on leash throughout Lake Front Park.
  • Dog Park Passes expire the last day of February of each year. 

*Required vaccinations include: Rabies, DHLPP, and Bordetella

**Homeowner's liability insurance must have at least $100,000 in coverage which includes coverage for injury/damage caused by the animal.

Type of Pass *
All-Access Dog Park Pass includes the use of the Mack Avenue Dog Park, the Lake Front Park Dog Park, and the option to walk your leashed dog(s) at Lake Front Park. Lake Front Park Only Dog Pass includes the option to walk your leashed dog(s) at Lake Front Park only.
